Is it true that before a U.S. Navy vessel goes to there's a series of certifications it has to have completed before going to sea?

That is correct, the USS Kinkaid DD-965 was decommissioned in 2003 and sunk as a target off Hawaii in 2004.
However one can assume that the problems listed within this report were not corrected and possibly led to the problems we are seeing now 28 years later.

I would hope so but 28 years is a long time to reinvent the wheel and do it the new and better way, what ever the hell that is. We should lean from history but continue to disregard much of it.

However we will not know if the same type of mistakes were made on the USS John S McCain DDG-56 until we see the report of the investigation. if ever. However there are a lot of items that a stick out like a sore thumb and I would hazard to guess are still prevalent in the fleet today, if not more so.

Cool Back to the old Navy (dungarees and chambray shirs)...

The report concerning the mishap of the KINKAID is, like any US NAVY report of/from a board of inquiry, outstandingly detailed, researched and accurate. It also places blame squarely on whom it belongs and the reasons it should be so. Part of the SOP for transiting the Strait of Malucca should be reading, digesting and implementing the recommendations given on this report.
These days there are far more distractions and, in my opinion, more disregard for rules, regs and safety. Conditions could be even MORE hazardous than in 1989. Nowadays everyone carries at least one electronic device that can distract one's attention from duty. In the Navy of today, improvements in instant communication with 'back home' have taken the sense of isolation away from serving at sea and so also distracts personnel from focusing completely on a job which demands all one's effort. Thus we see ships being run with lax rules toward basic seamanship, i.e watch standing, attention to detail, qualifications and time in rank.

That is correct, I used to run the "Radar Navigation Team Trainer." This was a pre-requisite the "Navigational Check Ride" that certified that the ship could navigate safely.

FYI the investigating officer, Admiral Chesbourgh was the XO when the ship was commissioned.

I 've heard that a number of the certifications these ships were required to have before putting had been expired and not updated.

Plus I have heard earlier this week the Navy removed a number of senior commanders as result of what happened.
