Sounds like the rocket-firing Brummbar or the Sturmtiger, but they were classified as assault guns. Beyond that, you get into things like the Karl Morser, which was never a tank and only barely self-propelled.

Looks like you're right. All I can find is the mentioning of the same tank built in 1945. It was apparently based on the T-34 medium tank (not sure how) but proved to have very bad cross-country performance and was consequently rejected for full scale production. I can't find any sources to cross check this info.

Does the main armament look like it could be a 37mm ZIS-19?

Here is quote from John Milsom's book "Russian tanks 1900-1970".
